Recent progress in PAH is based on new strategies for combination therapy and on the escalation of treatments to improve the significant morbidity and mortality associated with this disease. This case-based slide kit is designed to discuss the importance of monitoring patients to determine when and how to optimally alter their PAH therapy. Case 1 discusses a newly diagnosed PAH patient with a review of diagnosis, risk assessments, and initial combination therapy strategies. Case 2 discusses a high-risk PAH patient with a review of disease progression, risk assessment and treatment escalation options.

In this series of podcasts, the content faculty and one of their patients will discuss considerations and challenges in the prevention and treatment of HIV. Topics covered will include patient concerns, benefits and risks of antiretroviral therapy, and how patients have handled challenges in their HIV management.

The module will take the learner through a complete virtual patient tour starting at the time of symptoms of PAH through diagnosis, evaluation, and how the disorder is managed. Over the past two decades, research has vastly improved the understanding of the pathophysiology, diagnosis, and treatment of pulmonary arterial hypertension (PAH). While no therapies have been approved to target new mechanisms of action in more than 15 years, multiple phase 2 and phase 3 clinical trials of novel therapies are underway. Further, management decisions regarding practical issues must also be addressed. Current therapies for treating pulmonary hypertension aim to overcome the imbalance between vasoactive and vasodilator mediators and restore the endothelial cell function. Pulmonary Hypertension (PH) is currently divided into World Health Organization (WHO) groups based on the perceived etiology of the vascular insult: PAH (Group 1), PH due to left heart disease (Group 2), PH due to lung disease and/or hypoxia (Group 3), PH due to pulmonary artery obstructions (Group 4), and PH due to unclear and/or multifactorial mechanisms (Group 5). Accordingly, therapeutic interventions to address PH differ significantly between groups. However, this classification is less than perfect as there is not only considerable heterogeneity within each group but also overlap between groups. Emerging evidence on phenotyping PH may assist to address these limitations. This on-demand video presentation presents the latest research in diagnosing and managing Groups 2-5 in patients, including a discussion of the distinctions in specific groups – CTEPH, PH-ILD, PH-COPD.

PAH is characterized by increased pulmonary vascular resistance due to remodeling and occlusion of the pulmonary arterioles, resulting in right heart failure and death within 2-3 years of diagnosis if untreated. Despite advances in the diagnosis and treatment of PAH over the last two decades, the prognosis is still poor. Early recognition of disease, institution of effective PAH disease-specific therapy optimized for disease severity, and comprehensive care in specialized centers hold the promise of improving the outcome of patients. PAH is often misdiagnosed or unrecognized in clinical practice and delaying treatment options can worsen outcomes and survival. Diagnostic tests are needed to be done on anyone with unexplained dyspnea and suspected PAH. This on-demand presentation is designed using a case-based discussion on tools to improve the diagnosis of PAH and the importance of early referral.

Adolescents are disproportionately affected by HIV, accounting for approximately 1 in 5 new HIV infection in the United States. The expanded options for pre-exposure prophylaxis (PrEP) in adolescents provides clinicians with additional, effective tools to further reduce the risk of HIV transmission in this group. Once diagnosed, youth with HIV are the least likely out of any age group to be linked to care and achieve viral suppression once on ART.

One key strategy for ending the HIV epidemic in the United States is the rapid and large scale-up of pre-exposure prophylaxis (PrEP) provision by clinicians providing health care to HIV-uninfected persons at risk for HIV acquisition. Current recommendations encourage informing all sexually active adults and adolescents about PrEP, as well as offering PrEP to those at substantial risk of HIV acquisition. This activity will discuss the impact of PrEP on ending the HIV epidemic, benefits and limitations of new and future PrEP options, and strategies to improve sustainable implementation of PrEP.
